Maintenance/Repair Orders

On this page, you can access our 'Maintenance and Repair Order' form for completion and print-out.

• Please comply with the maintenance intervals specified by the manufacturer.
• Have all maintenance work and wear-and-tear repairs carried out by manufacturer-authorised workshops exclusively.
• Do not commission installation of accessories or any work related to improvement of the appearance of the vehicle.
• Use and sign the LeasePlan Repair Order forms exclusively.
• When you place an order with a workshop, please provide proof of your authorisation by presenting your TravelCard.
• Place orders for new tyres with our authorised tyre partners exclusively.

Have your vehicle serviced by a service and maintenance partner and you could be doubly rewarded – because all vehicle users who have their car serviced by one of the partners in our "Service & Maintenance Partner Network" between 1 March 2011 and 29 February 2012 will be entered in a prize draw.
